Purpose:
This
from is used by 90 days past due borrowers and borrowers who are
in non-monetary default, who did not apply for servicing or did
not accept servicing, to respond to notification that FSA intends
to accelerate their account. It is submitted in response to the
receipt of Form FSA-2525 which explains in detail all borrower
options.
