Thank you for participating in the Survey of Income and Program Participation. The
response(s) you gave are helping policy makers make informed decisions about the
status of government programs designed to aid millions of people around the country.
Because your household was selected to represent the U.S. population, your
continued participation in this survey is important. We appreciate your participation
greatly.
Enclosed you will find a change of address card and a postage-paid preaddressed
envelope. In the event that you or some member of your household have moved
or plan to move, please fill out the card and return it to us. Without updated
responses from all respondents each year, our vital survey information would quickly
become out dated. In return for your cooperation in updating your address, your
household will receive a $10 debit card.
As with your response to the survey itself, all information provided by you on the
change of address card will be held confidential under Title 13 of the United States
Code, which carries severe penalties for unlawful disclosure. This information will only
be used to contact you during the second phase of the survey.
