Thank you for choosing to participate in this survey, which will ask you about the
placement of the largest television in your home. We want to learn more about
television installations because children have been seriously injured and killed by
falling TVs. We hope to learn more about the placement of televisions in homes to
help us determine easy and effective methods for anchoring them.
It is important for us to get the most accurate responses possible on every
question, even for those questions that may seem unimportant to you. So please
think carefully about each question before answering. Also, to avoid influencing
other people's responses, please do not discuss this survey with anyone until after
the survey expires on [ENTER DATE].

The CPSC staff estimates that 42,700 people were treated in U.S. hospital
emergency rooms in 2006 because of injuries associated with television, furniture,
and appliance tipovers. Additionally, the CPSC staff has received reports of 180
deaths related to tipovers that occurred from 2000 to 2006.
To help prevent tipovers with televisions, furniture, and large appliances, the CPSC
staff recommends the following:
• Attach your TV to a wall mount, ceiling mount, or similar mount when possible.
• Place free-standing TVs on a sturdy, low-rise base as far back as possible. Avoid
flimsy shelves. Use the anchor devices that are provided with most large, flatscreen TVs.
• Place the electrical cords on TVs out of a child’s reach, and teach kids not to play
with them.
• Keep remote controls and other attractive items off TV stands or other furniture
holding the TV so kids will not be tempted to climb and grab for them and risk
knocking the TV over.
• Make sure all furniture is stable on its own. For added security, anchor chests,
dressers, TV stands, bookcases, and entertainment units to the floor or wall.
• Make sure free-standing ranges and stoves are installed with anti-tip brackets.
