Donor Histocompatibility (DHS) records are generated as soon as the donor feedback process
is completed in DonorNet® , or for a living donor, after living donor feedback is completed in
Tiedi®. The Donor Histocompatibility record is to be completed by the histocompatibility
laboratory responsible for performing tissue typing for deceased and living donors. This record
is not required for imported organs.

Provider Information
Lab: The lab reported in the Donor Feedback page in DonorNet will display. Verify that the
displayed histocompatibility laboratory provider number is the 6-character medicare
identification number, and that the displayed center code and center name are for the
laboratory responsible for the tissue typing of the donor.
OPO: The OPO reported in the Donor Feedback will display. Verify that the displayed OPO
provider number, OPO code and name are for the OPO responsible for the donor.
Donor Information
The following information reported in the Donor Feedback, will display in this section.
Verify that the information displayed for each field is correct.
Donor Name: Verify the last name and first name of the donor.
UNOS Donor ID #: Each donor is assigned a unique donor identification number by
OPTN/UNOS. This ID number corresponds to the date the donor information was entered into
the OPTN/UNOS computer system. For deceased donors, verify that the displayed 7-character
Donor ID corresponds to the Donor ID found on the match run used to allocate the organ(s).
Donor Type: Verify that the correct donor type is displayed.
Deceased indicates the donor was not living at the time of donation.
Living indicates the donor was living at the time of donation.
Donor Center Histocompatibility Typing
Donor HLA Typed: Select Yes if HLA typing was performed on this donor, and complete the
remainder of this record . Select No if HLA typing was not performed. If No or UNK is selected,
the remainder of this section is not required.
Date Typing Complete Class I and Class II: Enter the date that the donor was tissue
typed for Class I and/or Class II antigens by the histocompatibility laboratory. Use the
standard 8 digit numeric format of MM/DD/YYYY.
Target Source for Class I and Class II: Select the source of the target cells for Class I and/or
Class II antigens.

Typing Method Class I and Typing Method Class II: Select the antigen from the list. When
known, it is preferable to enter the split of an antigen rather than the parent. If the second
antigen at a locus is blank, select No second antigen detected. Only select Not tested when
the locus is not tested.

Recipient of a Living Donor Information
This section is only necessary for living donors.
The living recipient Name, SSN, Organ, Transplant Date and Transplant Center information
reported on the Living Donor Feedback are provided for reference.
Haplotype Match: Select the one choice indicating the haplotype match between the living
donor and the recipient.

Assignment of a .5 or 1.5 haplotype match means that either the recipient or the donor inherited
a "recombinant" HLA chromosome from his/her mother or father. If the other haplotype is
shared between the donor and recipient they will share 1.5 haplotypes; if it is not shared, they
will share .5 haplotypes. Select only one choice.
