SUBJECT: Health Resources and Services Administration Health Center Program Application Forms — OMB No. 0915-0285


This is a request for a non-material or nonsubstantive change to a currently approved OMB approval for the Health Resources and Services Administration’s Health Center Program Application Forms, for entities who are specifically participating in the School Based Health Center Capital Program. The current OMB approval under OMB No. 0915-0285 expires in 10/31/2013.

The Health Resources and Services Administration (HRSA) administers the School-Based Health Center Capital (SBHCC) Program, as authorized by the Patient Protection and Affordable Care Act (Affordable Care Act) ((P.L. 111-148), Title IV, Section 4101(a)).


School-based health centers (SBHC) are a major component of the nation’s health care safety net. SBHCs not only enable children with acute or chronic illnesses to attend school, but also improve their overall health and wellness through health screenings, health promotion, and disease prevention activities. The SBHCC grants will address significant and pressing capital needs to improve delivery and support expansion of services at SBHCs.

The following form includes the non-material/nonsubstantive change to the Health Center Program Application Forms: Proposal Cover Page, IC Part II, # 26. The changes are detailed below:

  • FTEs were removed from the SBHCC application since the Affordable Care Act does not require this information.

  • The section Green/Sustainable Design Practices were also removed from the SBHCC Proposal Cover, since it was not addressed within the Affordable Care Act.

  • The Financial Management and Control section were renamed to Resources/Capabilities, since SBHCC applicants and sites are not expected to existing grantees.


The program does not anticipate a change to current and approved burden estimates.
