Dear Dr. Schwab:

Attached is the combined request for 2012 pilot/special studies from the staff of the NCHS National Health and Nutrition Examination Survey (NHANES) (Generic clearance OMB 0920-0237).

The protocol describes plans to conduct the following:

1. Mobile Examination Center (MEC) Testing

   1. NHANES Human Papillomavirus (HPV) Prevalence Among U.S. Men Pilot Study (ages 14-59)

   2. NHANES Dental Fluorosis Imaging Project Feasibility Study (ages 6-19)

The total number of burden hours for these projects is 46 hours. This time was already budgeted and approved in line 2 - special study/pretest participants, of the original submission. The number of respondents is 355. The amount of time per participant varies by test component.

In addition, we would like to request an NHANES National Youth Fitness Survey (NYFS) remuneration change. Only a request to modify NYFS remuneration is being made. We are not requesting a change to any other aspect of the survey. Burden, interview questions, examination protocols, etc. remain the same.

All of these topics are described in detail in the attached supporting statement.
