To conduct economic analysis of the actual costs of carrying out policy- and environmental change-focused strategies at the community level. Results will inform governments and communities planning similar interventions in the future.

Study coordinated by and funded through HHS/ASPE.

Information will be reported by awardees, using a uniform, Web-based data collection instrument that allocates costs to each CPPW objective and the corresponding MAPPS strategies implemented by the CPPW site.

Case Studies of Selected Communities and States

Subset of Selected CPPW Awardees: 18 communities and 6 states

To describe the local context, planning effort, and implementation experience of the case-studied CPPW-funded sites. Reports will be used locally and nationally to understand implementation strengths and challenges and will be to improve immediate efforts and inform future efforts to implement community-based PSE change strategies.

Information will be collected through semi-structured interviews with key personnel at each CPPW site. Interviews will focus on how each awardee has implemented its strategies, the challenges encountered and addressed, things that have facilitated implementation, and lessons learned along the way.

Local Evaluation: Behavioral Risk Factor Surveillance

To provide site-specific estimates of the prevalence of risk factors that may change in response to community interventions.

Information will be collected by local CPPW awardees using the standard BRFSS infrastructure and locally established procedures.

Local Enhanced Evaluation: Biometric Supplement

Subset of 6 communities funded for obesity prevention through competitive award

To expand site-specific, individual-level data on height and weight (used to estimate BMI) and related biometric measurements. These awardees also produce supplementary observational measures of policy and environmental changes.

Data sources vary by community and are dependent upon each site’s community action plan, resources, and existing infrastructure.

Modeling/Simulation Study

Syndemics Modeling Study

Prospective evaluation of CPPW interventions, using a computer model, to forecast changes in key risk behavior, health and economic outcomes that will occur in the short- and long-term after the CPPW –funded period has concluded.

Information will be analyzed using a CPPW-specific version of the Prevention Impacts Simulation Model (PRISM), a computer model designed to estimate the potential impact of combined policies, systems, and environmental changes on cardiovascular disease and other chronic disease-related mortality costs. Data obtained through previously collected national and local surveys, such as the Census, BRFSS, and YRBS, will provide initial model inputs. Performance updates will be entered on a quarterly basis. This information will be taken from standard monthly call reports between Project Officers and awardees.
