Dear :

In a few days, your household may receive a telephone call to participate in a national study for the United States Department of Transportation.

This study gathers information on your experiences with the Nation’s transportation system involving security, reliability, and accessibility. The information will be used to evaluate and help improve our transportation system. The study results will be published on-line at www.bts.gov/omnibus. You can access this site at any time to learn more about the study. Your answers will be combined with answers from other study respondents to ensure that your confidentiality is completely protected.

Answering the study questions will take, on average, no more than 15 minutes. We depend on the voluntary cooperation of each household. When our representative calls about this important study, I hope we can count on your participation.

If you have any questions about the study, please call us at 1-800-xxx-xxxx. Thank you for your help.

Sincerely,

Steven K. Smith, Acting Director
