Dear Local Election Official:
The Federal Voting Assistance Program (FVAP), in coordination with the Defense
Manpower Data Center (DMDC), has selected your jurisdiction to participate in the FVAP 2010
Post-Election Voting Survey of Local Election Officials. You will receive the paper survey in the
mail around the week of November 1, 2010. The survey will also be available online around the
same time.
Following each presidential election, the post-election survey is used to produce a
congressional report mandated by the Uniformed and Overseas Citizens Absentee Voting Act.
The survey also helps FVAP improve the absentee voting process for uniformed service
members, their families, and citizens overseas by identifying voting issues experienced by these
groups and finding ways to addressed these issues in future elections.
If your address on this letter is incorrect, please provide us with a correct mailing address,
as well as an e-mail address to receive notifications about the survey by going to the following
site: https://www.dmdc.osd.mil/dodsurvey
You will be asked to enter your Ticket Number. Your number is: XXXXXX.
If you cannot access the Web site or if you have questions pertaining to the survey, you
may contact our Survey Processing Center by calling 1-800-881-5307, e-mailing
[email protected], or sending a fax to 1-763-268-3002. Please be sure to include
your Ticket Number in all communications.
If you have any questions or suggestions about this survey, or for more information on
the FVAP Post-Election Survey Program, please contact Mr. Allan White (FVAP), who can be
reached toll-free at 1-800-438-VOTE, or by using the toll-free numbers on the FVAP website
(www.fvap.gov), or by e-mailing: [email protected].
DMDC has set up a telephone line for anyone who wishes to verify the survey’s
legitimacy. Call 1-703-696-7427 for a list of current DMDC surveys. This prerecorded list
includes surveys only being conducted by DMDC, not surveys being conducted by any other
organization.
Thank you for your participation in this survey.
Sincerely,

Bob Carey
Director
