The Agency for Toxic Substances and Disease Registry (ATSDR) is doing a health survey of those who lived or worked at U.S. Marine Corps (USMC) Base Camp Lejeune or Camp Pendleton in 1987 or before. The Navy and Marine Corps are funding this survey.

The survey will ask questions about:

• when and where you lived at Camp Lejeune (or Camp Pendleton),

• your health,

• work history, and

• smoking and drinking habits.

This survey will help us learn more about ways to improve health and prevent disease in the future. The more surveys that are completed, the more likely the results will be valid. The survey and instructions are available at www.xxx.com. Your personal identification number (PIN) is xxx. This PIN is needed to access the survey over the internet. We also sent you a hard copy survey in the mail.

Please only complete one survey. You can complete the survey on line or by mailing us the completed survey in the stamped envelop that was included in your mailing. We would like to receive your completed survey by {2 weeks after date of this letter}.

You do not have to take part in the survey. There are no penalties if you decide not to take part in the survey. All answers you give will be kept private to the extent permitted by law. You will not be identified personally in any reports from this survey.
