About four weeks ago, we emailed you a link to a health survey. The link to the health survey is being sent to those who lived or worked at Camp Lejeune or Camp Pendleton in 1987 or before. To the best of our knowledge, the survey has not yet been returned.
We are sending this email again to encourage you to complete the health survey and return it to us. Your information is important for the success of the health survey.
We want to assure you that all answers you give will be kept private to the extent permitted by law. You will not be identified personally in any reports from this survey.
We hope that you will complete the survey soon. The survey and instructions are available at www.xxx.com. Your personal identification number (PIN) is xxx. This PIN is needed to access the survey over the internet.
We also sent you a hard copy survey in the mail. Please only complete one survey. You can complete the survey on line or by mailing us the completed survey in the stamped envelop that was included in your mailing. We would like to receive your completed survey by {2 weeks after date of this letter}. If for any reason you prefer not to answer it, please let us know by returning the enclosed stamped postcard and selecting the reason you do not wish to take part.
