The survey is being conducted by the Agency for Toxic Substances and Disease Registry and is funded by the Navy and the Marine Corps. Someone in your household should have recently received a questionnaire in the mail. We thank you if the questionnaire has already been mailed back to us. If it has not yet been returned, we hope you will consider completing the questionnaire and mailing it to us in the pre-paid postage envelope.
