Note to Reviewer
OSHA is making minor changes to the instruction on the following forms: Outreach Training Program
Report, OSHA Form 4-50.1; Outreach Training Program Report General Industry OSHA Form, 450.2; OSHA Training Program Report Maritime, OSHA Form 4-50.3; and OSHA Training Program
Report Disaster Site, OSHA Form 4-50.4.
OSHA is removing the reference to "Attachment B" under Item 5 of the Instruction Section to these
forms; and, under Item 9 replacing the phrase "of more than 50 students," with "that contained more
than students than allowed by OSHA policy.”
These changes eliminate unnecessary references and allow additional flexibility in the forms.
