Please see the screen shots for the new electronic version of the DS-1648. In an effort to comply with eGov regulations and the policies outlined in the Paperwork Reduction Act, the Department is transitioning to an online application form for all official and NATO nonimmigrant visa applicants.

Other than formatting changes in the electronic version to adapt to computer usage, and an additional question to provide an e-mail address, the electronic form essentially duplicates the information gathering questions of the paper version.

The burden time for this information collection was not affected by the change to this form.

To facilitate transition, upon approval of this new electronic application, the Department will request that the paper version of the form be discontinued.
