TITLE OF CONTEST: Chairman’s Commendation Circle Award

DESCRIPTION OF THIS SPECIFIC COLLECTION

  1. Basic contest description: The Chairman’s Commendation Circle Award recognizes people and groups who have contributed in an important way to the benefit of consumers by directly and significantly reducing deaths, preventing injuries, and improving consumer product safety. The award program focuses on individuals in organizations, businesses, state and local government and other groups.


  1. Objective for the contest: To recognize people and groups who have contributed in an important way to the benefit of consumers by directly and significantly reducing deaths, preventing injuries, and improving consumer product safety.


  1. Data elements to be collected: Nominator and Nominee Names, Organizations, Addresses, Email Addresses, and Telephone Numbers. Additionally, nominator can provide:


  • One (1) page (maximum) description of the individual or organization

  • Two (2) page (maximum) description of how, and to what extent, the nominee has contributed to improving consumer product safety

  • One (1) page (maximum) summary of any additional information that should be considered by the panel, including any grants received or other recognition of the nominee’s achievements in the area of product safety

  • Up to three (3) letters of support or three (3) pages maximum of other supporting documents


  1. Dates for the contest: Nominations must be received on or before July 11, 2014. Award recipients will be announced and the awards will be presented at a date to be determined.


  1. Eligible entrants/participants: Any individuals


  1. Number of winners expected: 1-5


  1. Selection criteria for winner: The Chairman will assess the recommendations and make final selections for the award based on a nominee’s commitment to product safety, the significance of the nominee’s contribution, and how consumers have benefited from the nominee’s contribution.


  1. Prize” or planned recognition for winner: Award winner(s) is (are) recognized by the Chairman of the CPSC, and listed on CPSC website. Awardees also receive a small plaque/statuette/trophy with their names engraved on its base.


  1. Other notes: The CPSC Chairman's Commendation Circle Award is not an endorsement of any individual, organization, company, service or product.
