Your 2010 Management and Organizational Practices Survey questionnaire, Form
MP-10002, is enclosed.
The U.S. Census Bureau is conducting a new Management and Organizational Practices
Survey to better understand current and evolving management and organizational
practices and to assist in identifying determinants of productivity growth.
Your response is required by law. Title 13, United States Code, requires businesses
and other organizations that receive this questionnaire to answer the questions and
submit the report to the U.S. Census Bureau. This same law guarantees that your
response is confidential and will be used only for statistical purposes.
We encourage you to complete this survey online at:

Your company's unique user ID and password is located on the front of the report form.
Please submit your report, within 30 days after receipt, via the Internet. If you
prefer, you may mail the form in the enclosed postage-paid return envelope within 30
days after receipt. If you choose to mail the form, please retain a copy for your records.
