DRAFT ADVANCE NOTIFICATION TO GRANTEES
Dear Healthy Start Grantee,
As you may know, Abt Associates Inc. has implemented a web-survey called the
Healthy Start Grantee Web-Based Survey. This survey is one of the data collection
efforts for the National Healthy Start Evaluation. We would like ALL grantees to
complete this online survey in order to ensure a complete assessment of the National
Healthy Start Program.
Please be aware that within the next two weeks you will receive an e-mail from us asking
you for your participation. The e-mail will include a link to the survey along with your
logon and password instructions. Please note that this email will come from
ADMINEMAIL. In order to avoid automatic filtering of your spam agent, we kindly ask
you to add ADMINEMAIL to your safe-sender list so that you are sure to receive the
information.
