The Community-based Care Transitions Program (CCTP) described in Section 3026 of the Affordable Care Act will run for 5 years with a mandated start date of January 1, 2011. This program provides funding to community-based organizations in partnership with acute care hospitals for the provision of care transition services delivered to high risk Medicare beneficiaries. The legislation provides $500,000,000 for the program. The goals of the CCTP are to improve transitions of beneficiaries from the inpatient hospital setting to other care settings, to improve quality of care, to reduce readmissions for high risk beneficiaries, and to document measureable savings to the Medicare program.

The legislation provides $500,000,000 for the program. The goals of the CCTP are to improve transitions of beneficiaries from the inpatient hospital setting to other care settings, to improve quality of care, to reduce readmissions for high risk beneficiaries, and to document measureable savings to the Medicare program. The success of section 3026 is critical to improving population health and therefore central to the Three Part Aim. Improving care transitions and reducing hospital readmissions is also one of the key areas for fast track delivery reform.
We had intended to use the Medicare Demonstration Waiver Application as the vehicle for eligible entities to apply. The Medicare Demonstration Waiver Application is approved by the Office of Management and Budget (OMB) and has been used successfully for demonstrations and pilots in the past. During departmental clearance, we were informed on or about
December 9, by the Office of General Counsel, that we could not use this longstanding approved vehicle because the CCTP is not a demonstration or pilot but rather a program. We are now seeking emergency OMB Paper Reduction Act review for the new application tailored specifically for the CCTP but modeled after the currently approved Medicare Demonstration Waiver Application. In order to be in compliance with the statutory start date of the program, the solicitation must be announced in the Federal Register by December 31, 2010, and therefore it is imperative that this application be reviewed under the emergency authority.
