Your Monthly Earnings
Usually, if you make more than the earnings limit, which in TaxYear is $AEA,
we have to hold back some of your Social Security. But if we know how much
you earned before taxes in each month in TaxYear, we may be able to pay you more.
The same is true of self-employed people. The difference is that we need to know
how many hours you worked in each month, instead of the amount of money you
earned.

NonWorkMonths

1.

If you worked for wages, put an “X” in the box under
each month when you earned $MEA or less. Be sure
to do it for the whole year.

PRIVACY ACT STATEMENT

The Social Security Administration (SSA) is authorized to collect information on
this form under section 205 (a) and section 203 (h) (3), (4) of the Social Security Act.
Giving us this information is voluntary. You do not have to do it, but we may not be
paying you the right amount unless you give us this information.
We use the information you give us to insure that we are paying you correctly.
However, we may share this information with another person or government agency
to manage the Social Security program or other programs that must be coordinated
with the SSA.
We may also use the information you give us in computer matching programs.
Matching programs compare our records with those of other Federal, State, or local
government agencies. Many agencies may use matching programs to find or prove
that a person qualifies for benefits paid by the Federal Government. The law allows
us to do this even if you do not agree to it.
Explanations about these and other reasons why information you provide us may be
used or given out are available in Social Security offices. If you want to learn more
about this, contact any Social Security office.

Reduction Act of 1995. You do not need to answer these questions unless we display

Sections 203(h)(3), (4), and 205(a) of the Social Security Act, as amended, authorize us to
collect the information requested on this form. We will use the information to ensure that we are
paying you correctly. The information you provide is voluntary. However, failure to provide us
with the requested information could prevent us from making an accurate and timely decision on
your benefit amount.
We rarely use the information provided on this form for any purpose other than for the
reasons stated above. However, we may use it for the administration and integrity of Social
Security programs. We may also disclose the information provided on this form in accordance
with approved routine uses of the Privacy Act, which include but are not limited to the
following:
1.

To enable a third party or an agency to assist Social Security in establishing rights to
Social Security benefits and/or coverage;

2.

To comply with Federal laws requiring the release of information from our records (e.g.,
to the Government Accountability Office, General Services Administration, National
Archives and Records Administration, and the Department of Veterans’ Affairs);

integrity of Social Security programs.

We may also use the information you provide in computer matching programs. Matching
programs compare our records with records kept by other Federal, State, or local government
agencies. Information from these matching programs can be used to establish or verify a
person’s eligibility for Federally-funded or administered benefit programs and for repayment
of payments or delinquent debts under these programs.
