Below are the following changes to the FEMA form numbers that are associated with OMB Control# 1660-0002, Title: Disaster Assistance Registration:

Justification:
FEMA is requesting additional versions of form numbers for FEMA Forms 009-0-1 and 009-0-2 instruments. The intent is to make inclusions to the primary form numbers in the current record. Four of the instruments have been given new form numbers. These changes do not change the hour burden or the scope of the collection but add four new form numbers to this collection.
