The U.S. Office of Special Counsel’s Disclosure Unit (DU) made minor modifications to Form 12. A copy in PDF of the draft is attached for review. Basically, we had an old phone number on the form which has now been updated. We made the information concerning the expiration date both current, and more explicit.
