Follow-Up Letter

Dear Resident,

Recently one of our field representatives asked you to participate in the National Health and Alcohol Study (NHAS), sponsored by the United States Public Health Service (USPHS). I am writing to ask for your help.

To better serve all people across the nation, the USPHS is conducting this national study on health-related issues. The goal of the study is to improve the health of all Americans.

The reason this study is being conducted is to provide information to researchers and policy makers so they can make informed decisions to determine how best to use available dollars and personnel to solve health problems. This affects you and your community to such an extent that I’m sure you would agree it’s important to have the most complete information possible.

As you may know, we cannot select someone else instead of you. Because interviewing every adult in America is far too expensive, we use statistical methods to scientifically select households to represent the total population. Each person asked to participate in the National Health and Alcohol Study is a representative of the many diverse groups of people who make up our nation. Without your help, others like you will be under-represented in the survey.

All of your answers will be kept strictly private. Your identity will not be known. The results of the study will be released in the form of percentages, totals, and averages in such a way that no individual’s answers can be identified.

You may be wondering how much of your time this will take. The interview takes about one hour, and most people have told us they find it interesting.

People completing the interview will receive a total of $90.

I have taken the liberty of asking our field representative to contact you again in the hope it will be possible for you to participate in this study.
