Fisheries Harvest Questionnaire
Prior to survey questions, the following information is obtained:

If YES Æ My name is ______________________. I’m from the University of Hawaii. We’re
helping the National Park Service conduct a survey of fishing in this park. Would you be willing to
answer a few questions about your fishing today? The questions will take about 15 minutes to
complete. All of your answers are voluntary and anonymous.
If NO Æ Thank you for your time. Have a good day.
