CY 2012 Formulary and Supplemental Record Layouts Changes
Formulary File Record Layout Changes:
1. A new field, Change_Type, has been added. This field will be used to specify
what type of change is being made to the formulary.
The permissible values are: ADD, DEL and UPD.
Excluded Drug File Record Layout Changes:
1. A new field, Gap_Coverage_YN, has been added to indicate if the excluded drug
is covered in the gap.
The permissible values are: 0 (No), 1 (Yes).
