SUBJECT: Request Burden Hour to be Added Back into SAMHSA’s Generic Customer Satisfaction Survey (OMB No. 0930-0197)


SAMHSA is requesting a change to the burden for our generic approval for “Voluntary Customer Satisfaction Surveys to Implement Executive Order 12862 in the Substance Abuse and Mental Health Services Administration.” SAMHSA is requesting that the approved 23,063 burden hours and the 90,000 responses be added back to OMB No. 0930-0197. On January 11, 2011 the 3-year clearance was submitted to OMB. SAMHSA mistakenly attached 2 ICs in Part 2 to show the total burden hours and responses that were requested. This resulted in a zero balance in which no ICs can be submitted.


SAMHSA is requesting that the change in burden hours and responses be added back to the March 4, 2011 approval as soon as possible.


SAMHSA looks forward to this correction so we may proceed with other data collections under this clearance.
