SUBJECT: Customer Feedback Survey on SAMHSA’s Publication: Medication –Assisted Treatment for Opioid Addiction: 2013 State Profile (OMB No. 0930-0197)


The attached materials are provided in support of our request for approval of a customer satisfaction feedback survey under SAMHSA’s generic approval for “Voluntary Customer Satisfaction Surveys to Implement Executive Order 12862 in the Substance Abuse and Mental Health Services Administration.”

This is the 10th submission under this approval and will impose an estimated annual average total burden of 17 hours, which would bring the total currently approved burden under this clearance to 14,462 hours for this data collection. The approved total burden hours for the Customer Satisfaction Survey (OMB No. 0930-0197) are 23,063 in which 14,404 hours were previously used in past data collections. The remaining total burden hours in the OMB inventory is 8,603 (126 first, 20 for the second, 29 for the third, 125 for the fourth, 389 for the fifth, 40 for the sixth, 5 for the seventh, 16 for the eighth, 56 for the ninth, and 17 for this request).
