APPENDIX B: TRIP SURVEY
E-MAIL INVITATION
Dear Panelist,
Earlier this year, we contacted you on behalf of the United states Coast Guard to talk about your recreational boat. I am
writing to request your participation in an additional survey.
The purpose of this survey is to explore your recent use of the boat.  This includes safety equipment on board, your
spending during the trip, and anything that might have happened on the trip, such as an accidental beaching.
It is important for us to obtain responses no matter how frequently people use their boats or how much money they
spend while boating.  Only with participation by everyone on the panel will the findings from the study represent
everyone in your state.
Your answers are completely confidential and will be used for statistical purposes only in accordance with the Privacy
Act of 1974. You are not required to answer any question that you feel is an intrusion of your privacy.
Go to the survey now by clicking the link below

[link]

If you have any questions or comments about this study, we would be happy to talk with you. Please contact SOME ONE
by phone at ###‐###‐#### or you can write to us at the address at the bottom of this letter.
Thank you very much for your help with this important study. Please return your completed questionnaire in the
postage paid envelope provided.
Sincerely,

An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it
displays a valid OMB control number. The Coast Guard estimates that the average burden for this report is 7.4 minutes.
You may submit any comments concerning the accuracy of this burden estimate or any suggestions for reducing the
burden to: Commandant (CG‐5422), U.S. Coast Guard, Washington, DC 20593‐ 0001.

Type I

Off‐shore Life
Jacket

Type II

Near‐shore
Buoyant Vest

This life jacket is
designed for
extended survival
in rough, open
water. It usually
will turn an
unconscious
person face up
and has over 22
pounds of
buoyancy. This is
the best life
jacket to keep
you afloat in
remote regions
where rescue
may be slow in
coming.

This "classic" vest
comes in several
sizes for adults
and children and
is for calm inland
water where
there is chance
of fast rescue. It
is less bulky and
less expensive
than a Type I,
and many will
turn an
unconscious
person face‐up in
the water.

Type III

Flotation Aid

Type IV

Throwable
Device

Type V

Special Use
Device

Flotation aids are
generally
considered the
most
comfortable,
with styles for
different boating
activities and
sports. They are
for use in calm
water where
there is good
chance of fast
rescue since they
will generally not
turn an
unconscious
person face‐up.

These are
designed to be
thrown to a
person in the
water.
Throwable
devices include
boat cushions,
ring buoys, and
horseshoe buoys.
They are not
designed to be
worn and must
be supplemented
by wearable life
jackets.

Special use life
jackets include
work vests, deck
suits, and hybrids
for restricted
use. Hybrid vests
contain some
internal
buoyancy and
are inflatable to
provide
additional
flotation.
