The Agency for Toxic Substances and Disease Registry (ATSDR) is conducting a survey

to learn more about the health of those who lived or worked at Marine Corps Base Camp

Lejeune or Camp Pendleton. ATSDR is a public health agency within the Centers for Disease

Control and Prevention. This project is paid for by the Navy and the Marine Corps. It will begin

when ATSDR sends you a health survey in about two weeks.

I am fully committed to this important project and strongly encourage you to participate.

The more surveys that are filled out and sent back, the more reliable the results of the survey will

be. Your participation is essential for this project’s success.

A few weeks ago Lieutenant General Frank Panter, my Deputy Commandant for Installations and

Logistics, sent a letter informing you of an upcoming health survey from the Agency for Toxic

Substances and Disease Registry (ATSDR). ATSDR is a public health agency within the Centers

for Disease Control and Prevention (CDC). As you may know, ATSDR is conducting a survey to

learn more about the health of those who lived or worked at Marine Corps Base Camp Lejeune or

Camp Pendleton.

Taking part in the survey is completely voluntary; however, I fully support ATSDR’s work on

this important project and strongly encourage you to participate. The more surveys that are filled

out and returned, the more likely the results will be useful. Please read the enclosed letter from

ATSDR for more details on completing the survey.
