BREWER’S NOTICE

TTB is amending this form to correct changes that were made previously. The changes have proven to be problematic and confusing for the applicant. Therefore, in item 10 we are changing the wording of the title for item 10, we are deleting “/Microbrewery” and the information in parenthesis under “Brewery,” and we are deleting the 1^st part of the information in parenthesis and changing the numbers in the information under “Brewpub.” Also, in item 12, we are adding “a” after “12” and adding “12b.” in front of the word “Title.” These changes are necessary to clear up any confusion, and will not affect the burden. We ask that you review and approve our request to make these changes.
