The following changes were made per suggestions from the Information
Returns Branch, email 2/9/2011
1. The form has been revised throughout to indicate that use of the
paper form is for one filer and that extensions for more than one filer
must be filed through the FIRE system and references to Transmitter
and Transmitter Control Code have been removed.
2. Box 3 has been shaded out and boxes 4, 5, and 6 have been
removed.
3. The Information Reporting Customer Service Site section has been
added.
