Forms are used to administer a
congressionally mandated grants programs and to comply with OMB
report requirements. FMCS uses the collected information to
determine applicant suitability, to monitor grant project status,
and for grant program evaluation. Grantees may use the forms as
management tools.
Program change due to Agency
discretion is a result of a reinstatement due to expiration of a
previously approved information collection which resulted from the
change due to violation of the PRA.
