SF 2800, Application for Death
Benefits Under CSRS The Department of the Treasury has directed the
Office of Personnel Management to inform applicants for benefits
that paper checks will no longer be issued to most payees. Those
who do not enroll in the Direct Deposit program so that their
payments will be deposited to their accounts in a financial
institution must contact the Department of the Treasury to obtain a
Direct Express debit card. SF 2800 has been revised to contain this
information and to inform applicants that it is illegal to use a
Direct Express debit card after the death of the owner of the card.
SF 2800 is needed to collect information so that OPM can pay death
benefits to the survivors of Federal employees and annuitants. SF
2800A is needed for deaths in service so that survivors can make
the needed election regarding military service. Every applicant who
uses SF 2800 should read SF 2800-1, Applying for Death Benefits
under CSRS. This brief booklet provides the general information
applicants need to understand what they are applying for.
