REQUEST FOR TRANSFER OF RESERVE CREDITS

This request for the transfer of reserve credits is submitted in compliance with the requirements of Marketing Order No. 984, as amended (Order). In executing this form, the handler respectively certifies to the California Walnut Board and the U.S. Department of Agriculture as to the correctness and completeness of their statement.

INSTRUCTIONS: 1. Complete the information below.

2. The form must be signed by both handlers and submitted in triplicate.

3. Following approval, the California Walnut Board will send a copy to each handler.

Note: No transfer of reserve credit will be approved by the California Walnut Board for either the receiving or transferring handler unless a completed form has been received. (7 CFR 984.56, 7 CFR 984.456)
