Information FOR NATIVE DIABETES WELLNESS PROGRAM DISCUSSION GROUPS

Project: Developing, Marketing, and Evaluation of Eagle Books and Youth Books for American Indians and Alaska Natives

Sponsored by: U.S. Department of Health and Human Services (HHS), Centers for Disease Control and Prevention (CDC), Native Diabetes Wellness Program

What this project is about: We are here to learn more about what middle schoolers think about a new book [INSERT TITLE] coming out that teaches kids about being healthy and how to prevent diabetes. Taking part in the group talk does not mean that you or your family are at higher risk for diabetes. We want to talk to your child because [INSERT TITLE] is especially for American Indian communities. So, we want to hear what your child thinks about [INSERT TITLE]. What you and your child tell us will help the Native Diabetes Wellness Program at CDC see how the books have helped children and how to make future books better.

Important things to know:

  • Your child will be in a small group, with 4 other kids their age.

  • The group will be held at the same place [INSERT LOCATION].

  • The group should last about 60 minutes.

  • Taking part in the group is totally up to you and your child. You and your child do not have to answer any questions you don’t want to.

  • You will receive $35 cash for your time and travel expenses. Your child will receive Eagle Books products about eating healthy staying physically active.

  • Someone will take notes during the groups. Your child’s name will not be written down or linked to anything that has been said. Only people working on this project will be allowed to see the notes from this talk. All notes will be kept in a safe and private place.

  • Everything you and your child say will be kept private, as allowed by law.

Neither you nor your child should be hurt in any kind of way because they took part in the groups. There are no direct benefits to you or your child for taking part in this talk. What your child tells us will help us understand what kids have learned from the Eagle Books.
