* The reporting period is a calendar day (midnight to midnight). Reports should be faxed to CDC each day by noon Eastern time. The fax number is 1-888-232-1322.

**Contact is defined as a family/household member, a person from the same facility (day care, LTCF), or a person in close contact (within 1 meter for >1 hour) of a suspect, probable, or confirmed case. Also, included as a contact would be a passenger traveling in the same compartment as a suspect, probable, or confirmed case (i.e., airplane, bus, or train compartment) ; any passenger, regardless of distance from case.



***Case definitions:

Suspect - A clinically compatible illness for whom H5N1 laboratory confirmation is pending and whose exposure is under investigation. Examples of persons whose exposures are under investigation are:

. A clinically compatible case with an epidemiologic link to a confirmed case

. A clinically compatible case testing positive for influenza A

. A clinically compatible case with travel to area with confirmed cases

. Severe atypical pneumonia in a previously healthy person < 65 years of age

. A person who is suspected of having pandemic influenza

Probable - A clinically compatible illness with an epidemiologic link to a confirmed H5N1 case and for whom no specimen suitable for testing was obtained.

Confirmed - A clinically compatible illness with H5N1 laboratory confirmation at CDC or at approved state health department laboratory.
