Delivery Satisfaction Survey

Invitation for workshop faculty to recite upon handing out the surveys to forum participants:
The following survey is being collected by the U.S. National Cancer Institute. The information collected will be used to
improve development, delivery, and follow-up of future Forums. Please answer the following questions thoroughly
and return your completed form to Forum organizers. Completion of this survey is voluntary, and will take 10 minutes
of your time. Your responses will be completely anonymous, and will only be used in aggregate to support the stated
purpose.
