Hello Grantees:

The EHDI team is charged with supporting our state partners in evaluating their efforts to address early hearing detection and early intervention by the development and enhancement of a surveillance system. To help us provide the highest level of technical support, we have created a survey to gather information about the technical assistance needs and current knowledge and skills among our grantees.

The survey, which should take no more than 15 minutes, is comprised of two sections.

Section 1:

  1. A self-assessment grantees can use to identify priority topics for technical assistance. The assessment includes most of the core evaluation competencies identified in the literature; few individuals will possess all of these competencies. This section will help EHDI prioritize its investments in evaluation resources.

Section 2:

  1. An assessment of the evaluation environment in which each grantee is working. This information will give the EHDI a better understanding of the challenges grantees with whom we work face as well as the resources they may rely on. It will also inform our support for local evaluation capacity building efforts.

You will find the survey at {LINK}. Please complete the survey by {DATE}. As you will see, we ask for your name and the state/territory/district for which you provide services.

Please note that while we’re using this survey to formally solicit technical assistance needs, we welcome your input at any point. You may provide comments directly to your liaison or to the CDC Evaluation Fellow, Brandi Knight, at [email protected].

Thank you, The Early Hearing Detection and Intervention Team
