About three weeks ago we sent a survey to you asking about your deployment and post-deployment experiences in Operation Enduring Freedom (OEF) and/or Operation Iraqi Freedom (OIF). To the best of our knowledge, the survey has not been returned.


The answers of veterans who have already responded include a wide variety of experiences, both good and bad, while deployed and after returning home. We think the results are going to be very useful for improving health care and other services for veterans like you.


We are writing again because is very important to hear from you about your experiences so that we get accurate results. Only by hearing from nearly everyone who, like you, was selected at random and invited to participate can we be sure that the results are truly representative of the experiences of men and women who served in OEF/OIF.


A few people have written to say that they should not have received the survey because they are not OEF/OIF veterans. If this applies to you, please let us know by answering “0” for question #3 on the cover of the survey and return it in the postage-paid envelope. We can then delete your name from the mailing list.


Please do not write your name anywhere on the survey. Protecting the confidentiality of your answers is very important to us. A survey identification number is printed on the survey so that we can check your name off of the mailing list when it is returned.


We hope that you will read the Project Fact Sheet and return a completed survey soon. But, if for any reason you prefer not to participate, please let us know by checking “no” for the first question and returning the blank survey in the enclosed envelope.
