Dear [LETTER TO BE PERSONALIZED BY EDC],

During the last two months we have sent you several mailings about an important research study we are conducting for the Department of Veterans Affairs (VA). Its purpose is to help the VA understand and respond to the health care needs of veterans who served in Operation Enduring Freedom (OEF) and/or Operation Iraqi Freedom (OIF).

The study is drawing to a close, and this is the last contact that will be made with the random sample of OEF/OIF veterans.

We are sending this final contact by priority mail because of our concern that people who have not responded may have had different experiences than those who have. Hearing from everyone who has been asked to participate help assure that the survey results are as accurate as possible.

We want to assure you that taking the survey is voluntary. Whether or not you participate will not affect your VA benefits or relationship with the VA. If you are not an OEF/OIF veteran, please let us know by answering “0” to question #3 on the first page of the survey and returning the blank survey to us. This would be very helpful.

We appreciate your willingness to consider taking the survey as we conclude this effort to better understand the different deployment and post-deployment experiences of men and women who have served in OEF/OIF. If you have any questions please feel free to call us toll-free at 1-866-912-8391. Thank you very much.

Sincerely,

Amy E. Street, Ph.D. Jaimie L. Gradus, MPH
