The purpose of this email is to provide notification that the project titled “Web-Based HIV
Behavioral Surveillance” is covered under CDC 308(d)Assurance of Confidentiality Control#
2008-NCHHSTP-036 titled "Assurance of Confidentiality for Surveillance of Acquired
Immunodeficiency Syndrome (AIDS) and Infection with Human Immunodeficiency Virus (HIV) and
Surveillance-related Data (Including Surveillance Information, Case Investigations, and
Supplemental Surveillance Projects, Research Activities, and Evaluations).
I have reviewed the protocol for this study and agree that this project falls in the category
of Supplemental Surveillance Activities, HIV Behavioral Surveillance because these
individuals would be at high risk for HIV infection.
