Supplemental Statement
The information in VA Forms 21-0960G-4 and 21-0960G-8 was initially submitted in the 60-day Federal Register notice on the VA Form 21-0960G-4, previously titled Intestines Surgical and/or Infectious Intestinal Disorders (Bowel Resection, Colostomy, Ileostomy, Bacterial and Parasitic Infections) Disability Benefits Questionnaire. The VA Form 21-0960G-4 has been separated into 2 forms because medically it made more sense to have the 2 medical conditions separated. This will also expedite the veteran's medical examination by only using one form for each condition. VAF 21-0960G-4 will now be named Intestinal Surgery (including Bowel Resection, Colostomy, and Ileostomy) Disability Benefits Questionnaire and VAF 21-0960G-8 will be Infectious Intestinal Disorders (including Bacterial and Parasitic Infections) Disability Benefits Questionnaire.
