Who is responsible for submitting. All owners, lessees, and charterers of a mothership vessel registered to an MS permit at any time in 2011

Complete all required questions. A complete EDC form contains responses to all required questions.

If you received or processed any fish using the vessel during 2011, all questions are required. If you did not receive or process any fish using the vessel during 2011, only questions 1 through 7 are required. If a required question is not applicable, write "NA" in the answer box.

Submit by September 1, 2011. Completed and signed EDC forms must be mailed and postmarked by or hand-delivered to NMFS no later than September 1, 2011. Mail or deliver to
