I am asking for your help with REACH U.S., an important study about health, diet, and exercise being conducted by the Centers for Disease Control and Prevention (CDC) and [GRANTEE] in your community. Results from the study are used by local health officials to improve health care programs.
In the next few weeks, an interviewer from the National Opinion Research Center (NORC) at the University of Chicago will call your household to take part in the study.
