[IF NO:] Your participation in the pilot study is being requested by CDC to help them determine whether the survey questionnaire can be successfully completed by municipalities with diverse organizational structures and to test methodologies to be used in the design of ongoing surveillance of policy and environmental supports. The pilot study findings will provide data on the policy supports that local governments are influencing, as well as valuable information on the best methods to use to successfully collect this type of data for a national surveillance system.

C. [IF ISSUE IS REFUSAL BASED:] Your participation in this study is very important to the CDC. The information you provide is invaluable in terms of understanding the current policies and standards enacted by local governments in regards to healthy eating and active living. CDC understands that your time is extremely valuable and that participating in a pilot study can be challenging. However, representation from your local government is extremely important. The participation of each selected community is important to ensure the completeness and accuracy of the results. Communities that are randomly selected cannot be replaced, and a high response rate is necessary to achieve valid state-wide estimates of health-enhancing policies and initiatives.


The survey takes on average 60 minutes to complete. We realize that it may be difficult to complete the entire questionnaire in a single setting. Therefore, we have designed the questionnaire so that you may log out and log back in at a later time with no associated data loss. CDC also encourages you to consult with colleagues or persons from other local government departments whom you feel can provide the most accurate data.
