The Agency for Toxic Substances and Disease Registry (ATSDR) is doing a survey to find out about the health status of those who resided or worked at U.S. Marine Corps (USMC) Base Camp Lejeune or Camp Pendleton in 1985 or before. ATSDR is a federal public health agency of the U.S. Department of Health and Human Services. The ATSDR Health Survey of Marine Corps Personnel and Civilians is funded by the U.S. Navy and Marine Corps.


The survey asks about when and where you lived or worked at Camp Lejeune or Camp Pendleton, as well as your work history and your health. By completing this survey, you will help us learn more about ways to improve health and prevent disease in the future. We really need your help to make this survey a success.


For your convenience, we are offering you two ways to complete the survey:


  1. Complete the enclosed survey booklet and return it to Westat, a contractor for ATSDR, by mailing it back in the envelope provided; or


  1. Answer the survey on-line at www.xxx.com. Your personal identification number (PIN) is xxx. This PIN is needed to access the survey over the internet.


Please complete only one survey – either on-line or by mail. We would like to receive your completed survey within two weeks.


Taking part in this survey is voluntary, but we hope you will participate. There are no penalties if you decide not to take part. All answers will be kept private to the extent permitted by law. You will not be identified personally in any reports from this survey.
