OMB change request memo

The purpose of this communication is to make a change request for OMB Control Number 0920-0909 (expiration date: November 30, 2014). Working toward the final implementation of the Diabetes Prevention Recognition Program (DPRP), usability testing indicates a need for minor, non-substantive changes to the following web pages (see below) in the Information Collection Request (ICR). The proposed changes will entail no increase in burden to either the applicant (respondent); in fact, usability testing revealed that the proposed changes will result in enhanced ease of use for the applicant.

Changes and/or updates are requested for the following:
