Setting: Interviewer will have information sheets and project identification available when approaching venue owners.

INTRODUCTION TO SURVEY

Interviewer Script: 

“Hello. My name is ____________ and I am working with CDC and the local Health Department to conduct a health survey in this community. We’re doing surveys of people in a variety of neighborhoods to better understand issues related to people’s health, their health behaviors, and how HIV (the virus that causes AIDS) may affect them.

The survey would take about 25 minutes to complete and eligible participants will receive $XX as a token of our appreciation^a. If possible, we would like to conduct some of these surveys here over the next 3-4 weeks. We are not trying to sell anything and our survey would not interfere with typical activities of your business patrons; we just want to learn about health behaviors of people in the community.

Would you be open to us coming here occasionally to recruit people for our survey?

• If no: “Thank you very much for your time! If you change your mind in the future, please feel free to contact us.” [leave contact information]

• If yes: “Thank you! We appreciate your cooperation. We would like to begin our survey administration here on [insert day]. We will check in with you briefly when we come.” (Provide venue owner/manager with project information sheet.)

ASSURANCES / ENCOURAGEMENTS

• We are not trying to sell anything.

• Our survey is voluntary, no one will be forced to participate.

• We will not interrupt business operations.

^a Tokens will be approximately $25 per participant (range $10 to $40). The exact amount for each city’s participants will be determined after discussions with local NHBS staff and community stakeholders.
