The information for this form is computer generated by TTB. The respondents are only required to enter information when the information generated is incorrect or incomplete. We have found it necessary to change the dates each year to reflect the current tax period. With this submission we are changing the dates as well as other minor revisions. Following are the adjustments that we are proposing:

• Change the OMB Expiration year (advance by one year)

• Change the format of the “TAX YEAR” and “2013 TOB” emblems on either side of the heading (pages 1, 2 and 3)

• Advance “2012 TOB” to “2013 TOB” (pages 1, 2 and 3)

• Advance “Tax Period” in the heading by one year (pages 1, 2 and 3)

• Correct fonts and formatting throughout

• Spell out “Failure to File”, “Failure to Pay”, and “Interest” in Item 13 (instead of FF, FP, I)

• Change Item 14 from “TOTAL TAX DUE” to “TOTAL DUE”

• Change due date in the statement below Section IV (advance by one year)

• Move the OMB info to the document header, move the form number, version date and page number to the footer area and take them out of the body of the form

• Update form version date in heading on page 4

• Remove all instruction numbers from page 4, inserted few breaks in the paragraphs for easier reading

• Update Tax Year in heading on page 5 (advance by one year)

• Update format of contact information box on page 5

We are making these adjustments at this time to prepare for this year’s mailing. Therefore, it is imperative that we get approval for these adjustments as quickly as possible.
