JUSTIFICATION FOR NO MATERIAL/NON-SUBSTANTIVE CHANGE
The following changes were made to the 1/2012 version of the form:
Reference to the Office of Taxpayer Burden Reduction (TBR) was removed in the first and last section to update to the new organizational structure which no longer includes a separate office of TBR.
Box 1 - Added "individual" as an option to ensure all taxpayer types are properly represented as a possible selection.
Box 3 - Added "individuals" (for same reason as box 1 above) and "all businesses" as an option; Removed the specific business boxes and replaced with two new boxes. "Particular Business Industry(s)" and "Particular Business Entity(s)" allowing the specific type to be a write-in. The business lines were reworded for ease and clarity of identification by the submitter.
