Parent/Guardian PEM Letter to School
PARENT/GUARDIAN PEM LETTER TO SCHOOL
Dear School Administrator,
This letter is to inform the school that our family has agreed to participate in a research study called Children’s Health after the Storms (CHATS). CHATS has been developed in response to concerns by parents and local community groups about potential health issues children faced after Hurricanes Katrina and Rita. CHATS will look at the health of children who are now ages 3 to 15 who lived in the region during and after Hurricanes Katrina and Rita.
As part of the study, my child will be wearing a Personal Exposure Measuring (PEM) device for about one week. It is important that the PEM be worn so that it can test the air quality throughout the day. This will include time at home as well as at school. The PEM will not interfere with daily activity. If necessary, it can be removed but should be kept nearby.
There is nothing that the school will need to do.
The study is being conducted by RTI International (RTI), a non-profit research organization and leader in children’s health research, with support from the Centers for Disease Control and Prevention (CDC) and our Gulf Coast partners, the Louisiana State University (LSU) Health Sciences Center and Coastal Family Health Centers. If you have any questions, there is a website that provides additional information. www.xxxxx.org You may also call a toll-free telephone number, 1-877-834-7088, to speak to CHATS project staff.
Thank you.
Sincerely,
